What is LDRT, and how does it work?
Compared with the doses required in cancer care, radiotherapy delivered in very low doses appears to have an anti-inflammatory and immune modulatory effects within the treated tissues. The net result of this is less pain and improved function.
As the radiotherapy doses used are very low, this treatment is not associated with any significant side-effects.
What conditions can be treated with LDRT?
The most studied condition for low-dose radiotherapy is osteoarthritis, particularly of the knee. There is also some data to suggest benefit in other inflammatory musculoskeletal conditions such as plantar fasciitis and tendinopathies.
Who is LDRT suitable for?
LDRT may be suitable for patients that have persistent pain despite a decent trial (3-6 months) of standard interventions such as education, weight management, physical activity and exercise, and topical and oral non-steroidal anti-inflammatory medications.
Who might not be suitable for LDRT?
LDRT is less likely to benefit patients with very advanced ‘bone-on-bone’ joint deterioration. Owing to a theoretical long term risk of malignancy, we also do not treat patients under the age of 50.
What are the key benefits of LDRT?
LDRT is an effective treatment for mild to moderate inflammatory musculoskeletal conditions. For patients that have tried conservative measures, but are not yet too advanced and requiring surgery, LDRT offers its greatest benefit. The treatment is very well tolerated with no or minimal side effects and does not interfere with future surgical procedures.

Before Treatment
What imaging or medical records should I provide?
Providing imaging evidence of disease, such as plain film x-rays, CT, and/or MRI is required for us to assess your suitability for LDRT treatment. During Treatment
What happens during an LDRT session?
During each LDRT session, you will be comfortably positioned on the treatment table, and the machine will deliver radiation from different angles to cover the target tissues and avoid unnecessary exposure. The treatment is completely painless and requires no anaesthetics or injections. Your treatment is carefully monitored by your care team to ensure accuracy and comfort.
How many sessions will I need, and how long does each take?
A course of LDRT is typically delivered in six sessions, spread over 2 to 3 weeks. Each session takes about 5-10 minutes. This is an outpatient treatment with no hospital stay required.
We typically follow up with patients at 12 weeks (~3 months) to assess response. if required, a further course may be offered at that time.
Is the treatment painful or uncomfortable?
The treatment itself is completely painless, but lying still on the treatment table during each session may feel uncomfortable for some patients. Our team will work to ensure you are as comfortable as possible throughout the process.
You will usually feel the same before and after treatment and can continue with all your daily activities throughout.
A small proportion of patients may experience a transient pain flare in treated joint following treatment. All potential side-effects will be clearly discussed with your care team.
Will I need to stay in the hospital?
LDRT is an outpatient procedure, meaning you can return home the same day after each session. There is no need for a hospital stay, allowing you to continue with your regular routine without significant disruption.
Can I bring someone with me for support?
Yes, you are welcome to bring someone with you for support. They can wait in the designated area during your session, providing reassurance and comfort throughout your visit.
After Treatment
What side effects can I expect after LDRT?
The doses used in LDRT are a small fraction of those used in cancer care, any side effects that we often associate with radiotherapy are largely irrelevant.
Transient skin reaction (redness, dryness) in less than 5% of patients. Transient pain flare after treatment is possible. Fatigue is possible, but considered rare.
Most patients experience no side-effects with this treatment.
How soon can I return to my daily activities?
You can continue your daily activities immediately after each session and throughout your treatment courrse. Apart from needing to attend the radiotherapy centre, there should be minimal disruption to your normal routine.
Will I need follow-up scans or tests?
Follow up is typically by clinical assessment for response to treatment. In the event of a poor response to treatment, then further investigations may be recommended.
What should I do if I experience unusual symptoms?

How will I know if the treatment was successful?
The success of LDRT is typically determined during follow up assessment 12 weeks after treatment.
Some patients require a second (and occasionally a third) course to achieve desired response.
If there is no improvement after 2 courses, then you are unlikely to respond to further.
